You can buy blanks from Bustin Bass Baits or Predator. I use auto air and createx paints.

This is a lot of fun. Here's what i would start with:
    -Decent airbrush
    -Createx Pearlized Paint starter set
-Devcon 2 Ton Epoxy (top coat)
-Make yourself a drying rack -made mine from an old grill rotisserie motor, a 2x2 with clothespins hotglued to it to hold the baits.
    -Some netting for scale patterns
-an old comb with wide teeth
-use manila folders to cut stencils
-very fine glitter in Gold, White/Pearl and Green

I was fortunate to already have a big air compressor with an air/water filter and the stuff for making my own drying rack. For patterns, open up your crankbait boxes, try and match some of them or use a catalog. And pour through everyone's work here and tackleunderground.com. Have fun!

Sources for blanks - same as above, but also Jann's Netcraft. They have some good ones too.
