For you cluster headache sufferers this article is by Dr Goadsby who I've had the chance to hear speak.

I had cluster headaches for over 17 years and I tried all kinds of pain pills, triptans and whatever else they would give me but nothing would stop them very well. Imitrex helps but it's hard on your heart and expensive so I don't like taking it. Then I was prescribed oxygen and it really works well with a re-breather mask and high flow of 15-25 lpm.
It's cheap, easy to use and no ill side effects. I used to kill a headache in under 5 mins using it.
My wife gets them every night at about my bed time.
Have had them for years. IMITREX - gone in twenty minutes
I have been getting them since I was about 15. They literally hurt me so bad that I would throw up and just couldn't function. I had them all weekend this weekend. Mine seem to be triggered by working with anything over my head where my arms are raised a lot.
I went to the doc about them and my neck hurting all the time. I found out I have two herniated disc and we think this is some of my problem with the migraines. I can usually tell in the morning if I will get one or not. I get them probably 6-8 times a month. Sleep is the only thing that will get rid of them for me.

I used to sleep on the bus coming home from school, run in the house, puke, go to bed and then sleep till the next morning. They run in my family - my paternal grandmother, my father, me........I just http://www.bassboatcentral.com/smileys/praying.gif they are done and my kids don't get them.
Now - here's what I've found...........usually SLEEP is the only way I can get rid of them or at least knock them down to a dull roar. Even a quick 30 min power nap helps me out tremendously.
Imitrex, Ralpacks...........both work for me - and almost immediately. BUT - I can't stand the way they make me feel. All constricted inside - and achy. BUT - if I've got a migraine bad enough, I'll take one.
Excederin and Mountain Dew...........one word - caffeine. That's what's the main ingredient in Excederin (either migrain or extra strength).........chase it down with a Dew and wait about 20min - my migraine is usually knocked down to a bad headache - enough I can actually funtion.
Don't waste your money on the Migraine strength Excederin..........same exact ingredients and dosage as Extra Strenght Migraine.........they just package it different and charge more money.
Man - I feel for all of you that suffer from Migraines..........there's nothing worse in this world. Well, I haven't had kidney stones yet......... http://www.bassboatcentral.com/smileys/Laugh.gif </td></tr></table>
This ^^^ To a T. Exactly the problems I have had, since I was a kid. The diesel fumes on the bus would trigger mine. Now all I take is caffeine pills and excederin. Sometimes I just have to bury me head in a pillow and try to sleep it off. I haven't tried the prescription stuff yet.
Have had them since I was about 14. I'm 50 now. I used to take Tylenol. But have since found Advil Liquigels. Pop a couple when I feel one coming on. Good to go in about half an hour. Havent had one in about a year now.
Two 100Mg Imitrex with me at all times. If those don't work I have injections. I know two in 24 hours is the max dose. When you have one you could care less what the max dose is. Two Topomax a day to try and keep them at bay.
Demerol/phenegran PRN....VERY LAST RESORT the rebound HA can be worse than the initial.
I get as many these days but no where near as severe as when I was in my 20's and 30's.
don't envy anyone else that deals with this.
Check out the Sumavel DosePro-- it is gas injected (no needle) Imitrex-- no pain --goes thru the skin--works quickly. Check with your doctor --it is of course by prescription
I finally broke down today and went to the dr. Got a script for imitrex, but the shot in the hip full of demerol and phenegren plus my other stuff they got me taking has numbed the pain quite a bit. This is day four and I would go outside and scream but am scared it would come back...
