My stepdad was living with my wife and I took him to his urologist about 4 years ago and some of the same questions, weapon, depressed, do you feel confused, emotions of a woman or man. I don't fault him as he was a great Army Dr. He asked for our reply and he typed exactly what my dad said. To the Hospital Administrator, ....

Have never been asked the weapons question but was asked if I was still had an active sex life. Was asked once when the nurse was going through all the questions if I've ever had thoughts on harming myself or others, I told her "myself no...but I've got a list"
