Daiwa Zillion handle but constantly backs off even after but replacement

I have a number of reels on which I swap out the handles for the 100mm Daiwa paddle handle. All of them work without issue except for this one JDM Zillion in particular. I’m aware that the bits can strip easily if over-tightened and simply assumed that’s what I did. However, after using several new nuts carefully tightened down, as soon as I set the hook, the nut gets loose and the handle almost falls off. Is it possible I’m missing a piece after disassembling? Is it possible the main handle shaft thread is stripped?

I’m considering sending this to someone, including DVT, but this shouldn’t be this complicated.

    What are your thoughts?

Sounds like stripped drive shaft threading. A small dab of super glue or blue lock tight might help hold it on for now if you want to fish it until a new driveshaft can be installed.

If you want to pay for shipping, you could send me the driveshaft and I can drill and tap it for a bolt.
