Never lost a cap myself. Have lost a very nice rod/reel when I turned around to answer my wife during a cast and hit the butt seat with the rod.

Had a couple blow off, but was able to turn around and get them. I just take mine off now and hang it on the shifter.

I be surprised if I've lost less than 20 hats over the years. I don't think I've ever broken out a new fishing hat because the old hat just wore out or smelled way too bad. I've been able to recover a few but I'd lose it somewhere down the road. Same goes for terry hand towels.

1 hat, and 1 rod. Needless to say I learned from both of those mishaps.

None. I ran Ranger's for many years so that may have helped.

Could have been blown off by a fierce crosswind
There's lots of decaffeinated brands that are just as tasty as the real thing.

My B.A.S.S. marshal hat blew off my head on the first day of the event on Lanier, my boater was nice enough to turn around so I could retrieve it.

Honesty about 6. Most of the time I can grab them before they sink. Sunglasses however......

None actually lost, but plenty have taken a dip. I've gotten pretty good over the years netting them out after I forget and it blows off my head.

Sunglasses, I have not been so lucky with - Twice I have thought they were on a strap and dropped when taking them off to put my readers on to re-tie (i'm old I guess).

Lost a couple when I switched from a Champion to a Ranger. And I was sitting on them.

Lost a favorite hat (iaconelli autograph on it) with a cap light attached to the bill. We were night fishing. The next morning my nephew (he was with his dad in another boat) asked me if I lost something? I told the story and he pulled my hat with the light still attached out of the boat compartment. The light must have shorted out and came on, they found it bobbing in the waves.

On average two per year. I stopped trying to find them with all the lake traffic just keep going.

I've lost quite a few and for some reason most sank like a rock. Now I run my life jacket buckle through it with my kill switch and never have to worry. My boat has strange air movement around the shifter when going down the lake so not a good idea to put it there. That said, my hat usually only blows off when I'm just going to ease "over there". I'll wear the nasty looking white hat I'm wearing now until it gets donated to the lake.
