Blinker style trim??????

Hey guys, I'm about to install a hydraulic jackplate to my rig. I have a single blinker style trim switch that was install when my hydraulic steering was installed. It has a collar with 2 bolts that just slip over. The blinker Has 2 bolts that connect to the collar on the right side. I want to add a 2nd blinker to the left side for the jackplate. Would anybody have a clue who would make this style. I do appreciate Any help!!

I have one on mine that is a single but have seen the dual ones from the same group. Mines a teleflex.
