what kind of 3/0 are you using? Ive used either a straight shank hook with the barbs at the eye along with using a 3/0 gammy EWG without any problems.

For heavy flippin' through mats I use Owner Screw-Lock. The beaver always stays put even punching through with a 1.5 ounce tungsten weight.
For t-rig flippin I use a regular EWG hook without problems.
Might try a 4/0 or 5/0 Gammie Skip Gap. I really like them
not sure what hook you are using, but you may want to try this. take a rubber peg it, wet it, run it through the eye of your hook and pull it to where it is tight. trim to where you have some sticking out on both sides of the eye. when you rig you bait, slide the head up over the eye. the rubber will hold the bait on the hook eye. toothpick will do the same thing, but is harder on your line than the rubber.
also, you can get the shrik wrap barbs that reaction innovation uses. nothing slides down those suckers.
3/0 Roboworm Rebarb Straight Shanks do the trick well enough for me. Dab of Super Glue on the hook eye could help a bunch too if fishign heavy stuff.

I use either a Gammy Superline Strait Shank (4/0) for general use and if I'm punching heavy mats, I would go with the Reaction Innovation BMF Flippin hook with the BMF barb (heat shrink tubing) that keeps the bait in place until you're done.
Also there's an article in this months BASS Times with Terry Scroggins that shows how he's modified a spinnerbait hook for flipping heavy cover...pretty cool.
