If you don't have a pattern to send them, Custom Mold won't do you any good. But they do show on their site on how to make a pattern.
You can make some measurements, but I belive #6 on the C&O link is what you want.

I think it is more like #7 but mine is a five hole and that is a 4 hole, not sure if it matters. I have the original but it's in 20 pieces. I don't have the lip on top like #6, but it's a great start and thanx for all the info. BBC kicks ass!
