Fury 25 threw a blade today

Anyone had any issues with a 25 3 blade fury throwing a blade? I was running about 55 mph today in the middle of the river 12-15 ft depth and felt some vibration in the steering wheel and immediately came down off plane and felt the vibration get worse. I trimmed up the motor to see a blade missing. Noticed what looked to be a air pocket in the material. Just checking to see if anyone has had a similar issue

Is it a stress crack from a nick in blade?
i run fury’s and was advised when making a high speed turn to make sure to trim in some. Lots of stress is placed on blade in high speed turns
