2012 yamaha 175hpdi trim tab came loose somehow and fell off. Should i replace it with another, leave it off, or do they make a flat one to cover the hole. Any suggestions?

All of the above. If you have hydraulic steering I'd either leave it off or buy a flat sacrificial anode to replace it with. And a new bolt.
Either will be fine but you should replace it. Aside from helping with corrosion,lack of one can cause prop blow out at hole shot.
