Put it in "park" plus put on the parking brake. Too many folks don't use the parking brake these days. IDK why...but LAZY comes to mind right away.

I've seen videos of trucks still sliding into the water if slimy or icy even with in park with the brake set. Sometimes that's when it's nice to have 4wd. Put it in 4 high to connect the front tires to the rear so that there's less chance to slide.

Article I read on it stated it slid down the ramp, as the ramp was covered in algae/slime. Apparently this ramp is cleaned about once a month and is a known problem there. So, the operator error was not one of neglecting to put in park/parking brake, but rather not observing the condition of the ramp.

Wonder if it came out of gear or the ramp was slick with something on it?

That thing went down like a rock. Unfortunately I have seen a lot of these videos and I have never seen one go down that fast.
